SRI Privacy notice – National survey of volunteers in response to COVID-19 in Wales

Introduction

SRI have been commissioned by Public Health Wales to undertake this survey. The purpose of this survey is to understand the role of community-led action across Wales in response to the COVID-19 pandemic. The findings will be used to help Public Health Wales and its partners better understand and inform their short, medium and long-term planning to improve population health in Wales in response to and in recovery from COVID-19. The survey results will help to inform future policies aimed at sustaining long-term support for both the third sector and community-led action.

Your participation in the survey is entirely voluntary, and you can withdraw at any time.

The personal data we process

We may process personal information that we collect from you.  Personal data means any information relating to you, and by which you can be identified. The specific information we will process may include your name, email address, phone number, and postcode where relevant.  

The legal basis for processing your personal data

Public Health Wales is the ‘data controller’ for this survey. Public Health Wales is an NHS organisation providing professional, independent public health advice and services to protect and improve the health and well-being of the population of Wales.

Public Health Wales has commissioned Strategic Research and Insight Ltd (SRI), a third-party independent research company, to collect and store the survey data on their behalf.

The legal basis for sharing your information with SRI is to enable the performance of a public task by Public Health Wales. While SRI will be processing any personal details on their behalf, Public Health Wales remains the controller of this information, and your information will only be used for the purposes above.

Sharing your personal data with others

SRI will never share personal information collected to any other third parties that are not mentioned here or in the survey without first explaining the purpose to you and obtaining your explicit consent. No personally identifiable information will be linked to your responses and only anonymous results will be shared with Public Health Wales. The only exception is where we have genuine concern for someone’s wellbeing, in which case relevant personal information may be shared with the appropriate authorities. If you opt in to be involved in future research connected with this project, we will share your email address with Public Health Wales but these will not be linked with your survey responses.

Retention and deletion

Personally Identifiable Information will be kept securely by SRI until the end of the project and will then be destroyed.
